How to refer to occupational health

For managers

Managers should make occupational health referrals using the online occupational health referral system, G2. The G2 portal has pre-set questionnaires developed by our occupational health team, so you know what information is required when you start the referral. You can also check the progress of your referrals from start to end.

Managers can refer their staff if they need advice on work related health issues, absence management, reasonable adjustments, ill health retirement, and other support to enable staff to return to or remain at work. All referrals will be triaged and allocated to the most appropriate member of the multidisciplinary team. This may be an:

Occupational health physician
Occupational health adviser (specialist community public health nurse)
Occupational Health therapist

To make a referral:


1. Please request login details for the system by emailing:
rf-tr.nlpss-ohsharedservice@nhs.net
Include your name, job title, department and manager’s name in the email.
2. Once you have received your login details, you can access the G2 system here to make the referral.
3. Please watch the manager’s guide video for details of how to make a referral once you have logged into the system.

To self-refer

Staff may self-refer for confidential advice on work related health issues. One of the occupational health team will respond within five working days to either signpost you to the most appropriate service (this may be an external agency, GP etc), provide telephone support or offer an appointment with a member of the occupational health multidisciplinary team.

To self-refer, please click here and follow the instructions on the page as outlined below:

1. Enter your email in the self-referral portal form to start the process. Then enter the captcha and press the green ‘request access code button’, which will send a code to your email.

2. Copy and paste the code from your email by clicking into the first box on the self-referral form to ensure you are using the correct numbers and letters.

3. The system will then ask for a customer code. This can be found on your local intranet site. Enter this code in the boxes provided.

4. Press continue to fill in the self-referral form.

5. Complete the form with the requested information, then press the green ‘Submit’ button
